What does shell rot look like?

My rescued slider seems to be doing well, but his shell has red splotches under the scales. Also, one of the scales is coming up. I have been rubing his shell with Terta Terrfauna's Vitashell. I want to help this little guy, buy I don't know what I need to do.

Thanks Toirt. It looks like another trip to the vet. My hubby won't be happy, but what can I do. I don't want to just let the turtle suffer.
